Plan terms
- Northbend Care is a membership agreement between you and Northbend Family Dental. It is not dental insurance, it is not a discount card accepted elsewhere, and it pays nothing at any other office.
- Fee: $34 a month for each adult and $24 a month for each child under 14, billed monthly on the day you join.
- Included each 12 months from the day you join: two exams, two cleanings, check-up X-rays once, one emergency exam, and fluoride varnish for children.
- Discounts for as long as the plan is active: 20% off fillings, crowns, root canal treatment, extractions and deep cleanings; 15% off whitening, bonding and veneers; 10% off clear aligners and retainers. Each discount is off the cash price on our price list.
- Term: month to month. It renews each month until you or we end it.
- Cancel any time with written notice by email or letter. No cancellation fee. Fees already paid are not refunded, and discounts end when the plan does.
- No waiting period, no deductible, no annual maximum, and no exclusion for conditions you already have.
- You sign the written agreement at the front desk or online before the plan starts, and you get a copy.
